I got this yesterday, I know it's a Collins, but what model is it?

P.S. - I need a nameplate if anyone's got a spare! :)

R-388 is the military version and has a slightly different antenna input circuit.

R-388 will have military inspection stamps somewhere (generally) and most are coated with MFP varnish inside. Nameplates are often missing on the R-388, evidently because they were sold by hams who got them via the MARS program and were not supposed to dispose of them.

Note the spot above the dial scale. where it says 5673, that is where the name plate should be.

Think this is a R-388

From the serial number it is probably Signal Corps contract 3357-PHILA-52 with an MFP date of 12/52
The ID tag would look like this one -
